For the 2020-21 school year only, any participant in athletics or marching band, in any grade level, who has not previously completed a preparticipation physical examination (PPE), and been cleared for participation, will be required to complete the medical history form as noted in current rule, as well as a PPE prior to participation in any UIL practices, games, performances, or matches.
As required under current rule, all students participating in UIL marching band and/or UIL athletic activities will be required to complete the UIL Medical History Form. Any “yes” answer to any of questions 1-6 on the Medical History Form will require a student to receive further medical evaluation, which may include a physical exam, and receive written clearance from a physician, physician assistant, chiropractor, or nurse practitioner before any participation in UIL practices, games, performances, or matches. Only those students with “yes” answers to those questions are required to receive written clearance through further medical evaluation.
Please note that this is only for the 2020-21 school year, and local school district policies still apply. Schools may still require physical examinations of students before allowing participation in these activities, and such decisions are under the jurisdiction of each local school district.

INCOMING 7th GRADE: Under the current UIL rule above, all incoming 7th graders must complete the Medical History Form as well as receive a Preparticipation Physical Examination.
~ THEY MUST HAVE A PHYSICAL TO ATTEND / PARTICIPATE IN ANY SUMMER WORKOUTS ~
Your incoming 7th grade child may not attend the June 8th workout, but they may attend any workouts after their physical examination offered by Mason ISD on Tuesday, June 9th, once they are cleared. If you choose a physical at a later date with another doctor, your child may NOT participate until they have received their physical and have been cleared by their physician.
INCOMING 8TH - 12TH GRADES:
~If your child participated in athletics last year and has a physical on file, they will not be required to get a new physical UNLESS they check "yes" to questions 1-6 on the new Medical History Form which is required for ALL ATHLETES / STUDENTS. If your child DOES check yes to questions 1-6 on the new Medical History Form and will need a physical, they must have it completed before participating in sports for the 2020-2021 school year, but they may continue to participate in summer workouts. If your child DID NOT participate in athletics last year, they will be required to get a physical and complete the medical history form. If your child is new to the district, we will need a copy of their physical from the previous school, and they will also need to fill out the new medical history form.
I have attached the MEDICAL HISTORY FORM and PHYSICAL EXAM FORM. To speed up the process, please print off both forms and have them filled out prior to showing up on Tuesday, June 9th. If you're unable to print these forms, we will provide hard copy forms here at the school on Tuesday, June 9th.
Just like in the past, Dr. Horne, Dr. Rochat, and Dr. Bray will offer physical examinations at the Mason ISD HS gym / locker rooms for $5. If you prefer to use your child's primary care physician, you may choose to do so. All social distancing requirements are in place per TEA / UIL Rules.
