1)      Students must read the information on the first page of the survey and give their consent to participate BY CLICKING THE BOX AT THE LEFT OF THE LAST LINE ON THE PAGE.

2)      Students should then click the NEXT button to proceed to the survey questions.

3)      If students mistakenly click only the NEXT button on page 1, a second page will come up asking them to agree to participate, again by clicking a box on the left of the last line.

4)      If for some reason a student does not want to indicate consent to participate, the student should simply close the browser window without answering any survey questions.
